A browser-only demonstration
Aveline Dental Studio is fictional. The enquiry controls validate information in your browser, then show a demonstration message. They do not send the information to a practice, a form service or a database.
What happens to entered information
The demo does not use cookies, localStorage or sessionStorage to retain your form entries. Entries remain in the current page until it is reset or closed. Please use invented details when exploring the form and do not enter medical information.
Without JavaScript, the form uses a local confirmation anchor. Personal fields have no name attributes, so their values are not added to the address bar. A selected topic may appear in the query string.
Hosting and external links
The hosting provider receives ordinary requests needed to deliver pages, fonts, scripts and images, and may process standard technical request information. The app includes no analytics, advertising pixels, embedded maps, external font loading or third-party form service.
The directions action opens an external map service only when you choose it. It points to Didsbury neighbourhood, not to an actual dental practice. That service applies its own privacy terms.
The articles link to general NHS patient information. Following an external link takes you to that organisation’s website.
Contact actions
The phone display is from an Ofcom drama range. Call, email and social controls open the demo note. The sample email uses the reserved .example domain. No contact action reaches an actual practice.
A real practice website
A real implementation would need the operator’s accurate privacy information, lawful data-handling arrangements and a properly secured enquiry route before collecting patient details. This demonstration does not collect enquiries.
